Sidexis 4 Review
More than just imaging software
Our Verdict
Sidexis 4 is the imaging software arm of Dentsply Sirona, the world's largest dental equipment manufacturer. If you have bought a Sirona sensor, CBCT, or panoramic unit in the last decade, you are almost certainly running Sidexis already. It is the default imaging platform for one of the biggest hardware ecosystems in dentistry, and it does that job well -- centralizing 2D and 3D images in a clean interface with strong diagnostic tools. The question is whether being the default makes it the best, or just the most familiar.

What Is Sidexis 4?
Sidexis 4 is Dentsply Sirona's imaging software platform for capturing, processing, storing, and analyzing dental radiographs. It handles everything from periapicals and bitewings to panoramics and full CBCT volumes -- all within the same interface. The software has been the centerpiece of Sirona's digital imaging ecosystem for years and integrates with over 250 practice management systems worldwide.
The platform is built around a modular architecture. At its core is image acquisition and storage, but it extends into 3D visualization with multiplanar reconstruction, implant planning tools, and integration with DS Core for cloud-based backup and sharing. The Lightbox feature lets you work with different file types -- X-rays, intraoral scans, photographs, and PDFs -- in one centralized view, which is genuinely useful for comprehensive case presentations.
Dentsply Sirona reported $3.7 billion in revenue in 2024, making it the largest pure-play dental company in the world. Sidexis 4 is a cog in that machine -- designed to keep you buying Sirona hardware and keep your data flowing through their ecosystem.
Who Is It For?
Sidexis 4 is primarily for practices that have already committed to Sirona's hardware lineup -- Schick sensors, Orthophos panoramic/CBCT units, Axeos CBCT systems. If you are running Sirona equipment, Sidexis is the path of least resistance. The integration is seamless and purpose-built.
It also works well for multi-location groups that want a standardized imaging workflow across sites, especially if they are standardizing on Sirona hardware. The DS Core cloud layer enables image sharing between locations and with specialists without burning CDs or emailing DICOM files.
General practices doing both 2D and 3D imaging benefit from having everything in one platform rather than juggling separate software for sensors and CBCT. Specialists doing implant planning will appreciate the built-in measurement and visualization tools, though dedicated planning software like coDiagnostiX or Blue Sky Plan offers more depth.
Key Features
Unified 2D/3D Imaging: Capture and view periapicals, bitewings, panoramics, cephalometrics, and CBCT volumes in a single interface. Compare Mode lets you place different X-rays side by side for tracking changes over time.
Lightbox: A centralized workspace for all patient imaging files -- not just X-rays but also intraoral scans, clinical photos, and PDFs. This is particularly useful during treatment presentations when you want to pull up everything for a patient in one view.
3D Visualization: The CBCT module includes multiplanar reconstruction (MPR), cross-sectional views, panoramic reformats, and measurement tools. Implant planning workflows help identify anatomical landmarks like the mental foramen and inferior alveolar nerve.
DS Core Integration: Cloud-based backup and sharing that automatically uploads 2D and 3D files from Sidexis. Enables remote access to patient imaging data and simplified referral workflows with specialists.
PMS Integration: Connects with 250+ practice management systems through TWAIN and bridge interfaces, though the tightest integration is obviously with Dentsply Sirona's own products.
Pricing
Sidexis 4 is typically bundled with Sirona hardware purchases. When you buy an Orthophos CBCT or a Schick sensor, the software license comes as part of the package deal negotiated through your dealer (usually Patterson or Benco). Standalone licenses for practices wanting to add Sidexis without new hardware are available through dealers on a quote basis.
This bundling model makes it hard to evaluate Sidexis on cost alone -- you are usually evaluating the total hardware-plus-software package. DS Core cloud features may carry additional subscription fees depending on the tier.
Pros
- Seamless integration with the entire Sirona hardware ecosystem
- Genuinely strong 2D and 3D imaging in a single unified interface
- Lightbox feature for centralizing all patient imaging files
- Broad PMS compatibility with 250+ systems
- DS Core cloud backup and sharing is well executed
- Backed by the largest dental equipment company in the world
Cons
- Effectively locked into the Sirona ecosystem -- limited value if you are running non-Sirona hardware
- On-premise installation requires server infrastructure and IT maintenance
- Software updates are slow to roll out and can require retraining
- Referral workflows for sending scans to radiologists are clunky
- Learning curve is steep for new users without prior imaging software experience
- Pricing opacity -- everything is bundled and dealer-negotiated
The Bottom Line
Sidexis 4 is an excellent imaging platform if you are already in the Sirona ecosystem. The unified 2D/3D workflow is polished, the Lightbox feature is genuinely useful, and DS Core adds cloud capabilities that bring the platform into the modern era. But this is very much a walled garden play. If you are running mixed hardware or considering non-Sirona equipment, you will get more flexibility from DEXIS, Apteryx, or even SOTA Cloud. The lack of transparent pricing and the slow update cadence are frustrations, but for the thousands of practices already running Sirona equipment, Sidexis 4 remains the obvious choice -- and a competent one.
